Forms Not Working

Our clients are filling out one of forms (that has NOT had any changes made to it) and we are not getting the submissions. How does it go from working perfectly fine for years to not working? What do we need to do?

I am getting ready to post on our website that the forms are not working. They seemed to work for a few days - but once again they are down. I sent 2 test forms today (they each go to a different email address) and neither have come through. When we call client's to apologize and say we didn't receive anything - they are getting upset and refusing to send in their order again.
