www.allegiancepwds.com I spoke to help desk and we both did a test and several others in their office from phones and computers and it worked for them 4 times.  They said people would have to clear their cache or change browsers.  We used Edge and Chrome to do it.

@amy6821​ Hi Amy, I tried to fill out the form on your website and received the attached error message (see screenshot). Please review the recommendation provided. Additionally, I would like to know if other users who encountered the 500 error while filling out the form are using the same internet providers. 

I don't see the error on screenshot just says it was submitted.  I however just finished creating an all new application on the site so perhaps that will fix the problem.  If you could try again at your convenience.  Thank you
