Database collection forms

Is it possible to add a database collection form on a Homestead website, perhaps through HTML code? I want to build a contact form: When a visitor submits the information, instead of generating an email, it builds a database. For example with the fields:
First Name/Last Name/Company Name/Email Address
with each field building in a table or spreadsheet.
I have embedded functional programs in Homestead websites in the past, just wondering if there is one for this application. Thank you!
