when someone submits a form they get an error page

Recently when you submit a form on our website you get an error page instead of our Thank You page. Customers keep resubmitting thinking their form didn't go through or calling us complaining. We are receiving the forms via email. At least it seems like it. Hopefully we are. Please fix. Thank you.
