Question about submitting forms

I have created a page where I want to have visitors send me information using the form check mark boxes, single line text boxes and the dialog box. I have 21 check mark boxes, 6 single line text boxes and 1 dialog box. My question is can I stick one submit button on the page and link it to my email address and this will submit the responses to all of my check mark boxes, single line text boxes and the dialog box? If not how do I get all the responses submitted?
