Forms not working

No notification (not working) coming from Homestead in regards to <forms> on my site. http://www.aalondon.org I have filled out this form a couple of times a couple days apart to test – nothing comes back to my email account.

This form and others similar to it have been on my site for a number of years and I have not edited any of these forms for over 30 days.

The form was originally as http://www.aalondon.org/visitorsRequest.html but had to place a notice on the page that it wasn’t working – so I copied the page to http://www.aalondon.org/visitorsRequest-2-BE-REPLACE.html
