Submit Button Not Working

When someone tries to submit information from the page, the information does not go to the e-mail address listed. They get a page saying the information has been sent, but then on the next page, the "Back" button results in an error saying the page cannot connect to our site. The information is never sent to the e-mail address.

It's happening on two of our pages, but we first noticed the problem on page 8 of the Animal Rescue Fund website's county fair chart signup page (arfund.homestead.com/ARF8.html). We have the "Submit" button on another page also (18), and e-mail replies from there never get to the supplied e-mail address either. We tried placing a "Contact Form" on the page, but replies from it also never made it to the provided e-mail address.
