Simple Store - adding an 'application' for customers

I am a teacher who offers camps over the summer.  I want to add the simple store to my website so 'customers' can register and pay online with a credit card.  My dilemma: is there a way to combine the simple store checkout procedure to add my camp application?  So parents can fill everything out at one time?  That way I'll have all the camper info (student name, grade, emergency contact, etc etc...)
Is this possible??

There isn't really a way to integrate an application to the store, but we may be able to figure something out. What do you use for the application?
Currently, parents (customers) visit my website, download & print the camp application (which is a pdf), fill out the application and mail it to me via snail mail with a check.

I wish to streamline this process so parents can pay & register their child (all online) in one easy step.  So on the checkout page in the simple store where they would be filling out their credit card number, expiration, etc - I would also need boxes for them to fill in things like:  child's name, child's age, child's address, emergency phone number, etc etc....... Is this possible??

This will not be exactly possible, though you could create a custom form for them to fill out as the application and set the submit button to a custom Thank you page, which would be the store page. They could then add the item to the cart and make a payment. There is not why to integrate the two events, but if you receive the email form, but no notice of a payment then you will be able to check with the parent.
