Mailing List Form Showing Up on ALL Pages...Help!

I have a Mailing List form on the Home page positioned where I want it. Unfortunately I am unable to remove it from the "other" pages in the site. Is there a way to do this? Check box options for items to show up/not show up on a page maybe? (http://MyHarvestFreshFarms.com)

Hello,

This was definitely a weird one since I didn't see it was set to show on all pages. However, what I did to fix this was copied it first,(ctrl+c)  then deleted it. Upon deleting it did say it was on multiple pages, I clicked ok. Then I just pasted it (ctrl+v) back on the home page and it only displays on that page. Give that a try and let me know if that fixes it on your end also.

One other quick (hopefully) question...
Is there a way to add a check box to a
form? Like "agree to terms" and "subscribe"
to mailing list, on one form that is "Submitted"
via the form...?

Perfect. Unfortunately not, there is not a way to add a checkbox to a form.

If you do need that feature you may be able to get that using JotForm :)
