How do I edit a custom form I created on SiteBuilder years ago?

Several years ago I set up our "Request a Quote" page on www.renovationsetc.com with a custom form. That has been working well. The responses are forwarded to my client's personal email and to mine. We both receive a "Your Web Site Received a Form Submission" email that include the responses. It has been so long that I don't remember how I created the form. How do I make minor edits?

It appears the site was created in the desktop version so open that up and log in to the site.

Forms are created by using the options under Communication in the middle menu on the editing page. To change the form you would click on the item you want to change and the Properties Editor will open to that particular item in the form.
