How do I combine custom form elements?

I see how to add form elements, but how do I create the form those elements are to be associated with? All I end up with is unrelated form elements on my web page.

Hi David2455,

The submit does this automatically.  When someone clicks submit, all form elements on the page are submitted in a single email.

This also means that you can only have 1 custom form per page.

So I create a simple colored box behind it to simulate a form, add a text box for a form title, etc?  Seems a bit crude, but I can make that work.  Thanks.

That ius the way it works.  We are hoping to improve on the custom forms in the future. 

When you add a text box, you will see on the right side that there is a name, that will say check box 1.  If you rename it with the relevant data that element will be asking for, you will receive a better email.
