I have created a patient paperwork form using Web Builder, but when we receive it in our email, it is not in the same order that it appears on the webpage. For instance, the whole top section is located halfway down the information when it is emailed to us. So in order for find the patient's name and address, etc we have to scroll way down to page 3. Is it possible for you, on the back side, to arrange the order in which they are emailed to us?

And as a follow up, can the form that's emailed to us only contain the items that were filled out? In other words, if the patient does not have any medical conditions and didn't select any of them, can it omit those in the form that is emailed? It would turn a 4 page print out into 1 page in some cases, which would be great.

Hello,

Unfortunately, there is no way for us to change the order on our end. If sends the information in the order that the form fields were added. I would recommend you select the form elements, cut and paste them onto a different page, then one by one add them back onto the original page in the order you want them to display.

There is not a way to omit form options.
