Are you checking the email @yahoo.ca for these? You can try to delete the form and add it back, but yahoo is pretty notorious for blocking messages at times. You could ask them if it is possible to whitelist a sender under their system.

Did you try deleting the form and adding a new one to the page. If that doesn't solve it, it is almost certainly that yahoo is blocking it. The only option would be to try a different email address. We can't unblock it. Yahoo would have to do it.
