cart does not empty after order is placed

I've had this problem many times in the past, you have worked on this and thought it was corrected, but it is still happening.  The issue is that often (I don't know if it's every time) after a customer completes their order (it goes through PayPal), they are redirected back to their cart and it still has all of the items in it.  So then they think the order did not go through and place that order again - causing frustration for them and for me because now I have to spend the time to refund their money and contact them with apologies.
