am having problems deleting one of the unwanted contact forms on the left hand side of my website I need help to remove it...

As stated above the title needs to help me remove the contact form on my website at the left handside...my website is www.hdpsl.org

I'm not seeing a contact form on the left on any of the pages. Perhaps try refreshing your view in the browser.

The only contact form is on you DONATE page. And, of course, the "Contact us" form at the bottom of each page.
