Alligning and labeling fields in custom email form

Is there a way to insert a table into the custom email form rather than manually dragging and trying to align the text boxes? 

Also, is there a way to insert a caption label next to the boxes so that I don't have to insert a text box next to it such as "name" "street address" etc?  I enter text in the box in the properties, but then it lets me just start typing something right next to the label so it will show up in the box as "NameAngela."

A google search of "HTML forms" or "HTML forms generator" will get you pages and pages of sites offereing free, customisable forms.. Find one that suits your needs and then copy the code and insert it into your page.  ONce you have chosen a suitable form you can check how it would look on a HTML viewer site before you put it on your page. I use two sites to check any code that I want to use.. http://htmledit.squarefree.com/ and to tweak code http://codebeautify.org/htmlviewer/
