Adding an Upload Photo Option In Form

I have created a form that works well. I want to add an option for the user to optionally "Add a Photo". I have the html code to upload a file, but it does not seem to save. Any help out there?

Hello,

Could you explain what you mean by "it doesn't seem to save"? The reason I am asking is because I can see the "Choose File" option on the published site.

Sure - I have 4 or 5 other fields e.g. name, email, etc.  They upload to the form file just fine. When I try to add in the option for adding a photo, nada. No picture or signs of a field that looks like it wants to accept.

I see. The entire form would need to be created via HTML if you wanted the image upload to be included in the form submission. Basically, there is nothing connecting the HTML you added to the submit button for our form. You can easily create one using a program like Jotform.

Liz - Spent the entire day trying to get a working form. Still no luck. When I submit the form the notice comes back ok when I press the next button I get the same error message. What is wrong?

What page are you experiencing this on? It looks like the page that I saw before no longer exists. Did you create a new one?
Also, if you are using partial HTML in the form through our builder, I have no way to get the HTML working. We cannot troubleshoot 3rd party code. You would need to create the entire form through a program like Jotform.
