Adding a Google Doc (using a hyperlink) Form to Website

We are trying to add a Google Doc Form on website, which will allow interested parties to fill out pertinent information electronically and submit. We have the Goggle Doc Contact Form made, but can't get it to show up on website. Do we insert as a Html? If so, what are the steps?

If you are able to extract the HTML embed code for that google doc form you can place that on your website by using a HTML element box. https://community.homestead.com/homestead/topics/sitebuilder-plus-add-html 
]I am not sure on the process to obtain the HTML code from the google form, apologies.
