"Request Info" box is not working

I was studying my stats and realized that a number of visitors have gone to the "request info" box, but I have not received any requests.  I checked and had my husband check from a different computer and  the "request info" things I added to my website last week do not work.  The one that had been there previously does.  I looked at the set up box and it looks like everything is done correctly...  Help!!
